The transport equation for the nuclear component of cosmic rays in the interstellar medium containing randomly distributed giant molecular clouds is obtained. It takes into account the finite transparency of the clouds for diffusing relativistic particles and the screening of the internal cosmic ray sources. The equation is used to improve the standard model of cosmic ray propagation in the Galaxy.
