We regret to inform the community that the object identified as the quasar QSO 0111-2838 in the paper (AJ, 108, 1147 (1994)] was misidentified and is actually a field galaxy at low redshift. Due to an error in a flat-fielding procedure, its spectrum was switched with that of another candidate which can be identified as the quasar BRI 0241-01. Most of Sec. 3 should be disregarded, except for those portions discussing the spectrum (Fig. 8), which now apply to BRI 0241-01. Figure 7 should be disregarded and the figure caption of Fig. 8, p. 1155, should read "An 1800 s low resolution spectrum of BRI 0241-01, z = 4.042,... ." The entry for QSO 0111-2838 in Table 1 should also be disregarded, and its entry in Table 2 now pertains to BRI 0241-01. This now brings the number of BRI quasars rediscovered during the course of the project up to seven. Those desiring finding charts or coordinates for BRI 0241-01 should contact julia@astro.caltech.edu.
