1) The signs of all terms on the right-hand side of equation (4) should be reversed. The results are unchanged, since expression is equal to zero in spherical symmetry.
2) The signs of all but the last term on the right-hand side of equation (9) should be reversed. The results are unchanged, since the correct expression was used in the calculations.
