The equations governing the `box model' approach to diffusive shock
acceleration are derived and generalised to apply to acceleration at
relativistic shocks. The difficulties encountered in describing the
particle spectrum close to the point where losses balance gains are
discussed in relation to the question of `pile-ups'.
