The traditional formulation of quantum mechanics based on the Heisenberg picture or on the Schroedinger picture is reviewed critically, noting that the covariance of the relevant equations is destroyed by a unitary transformation. A covariant form of quantum theory that is valid ab initio in any frame of reference is outlined on the basis of the principle of fundamental covariance. Four fundamental laws are proposed: (1) Heisenberg's commutation rules, (2) the Heisenberg equation of motion for an operator, (3) an eigenvalue equation, and (4) an equation of motion for an eigenstate.
