In the process of preparing the paper for print, Table 4 was lost and erroneously replaced by Table 5. The reference to Table 4 in § 3.4. is correct, and refers to the (omitted) Table 4, which is given below.
The present Tables 4 and 5 should correctly be numbered 5 and 6. The mention of Table 4 in § 4.1 should read Table 5. The reference to Table 5 in Appendix B should read Table 6. The omission of the correct Table 4 does not affect any of the conclusions of the paper.
The Press sincerely regrets this error.
