Various causes of loss of spectrograph efficiency, which arise in
multi-object fibre-fed spectroscopy and in the instrumentation of very
large telescopes, are discussed. A new form of spectrograph camera is
described which provides some mitigation. A numerical example is given,
illustrating, in particular, reduction of central obstruction.
