Line-driven winds are subject to a strong radiation-hydrodynamic instability. We discuss the linear stability analysis and numerical simulations of the fully developed wind structure. The latter show sequences of strong reverse shocks, and two different families of clouds which mutually collide. Possible applications are the X-ray emission from O stars and the formation of dense clouds in broad absorption line quasars.
