Much attention has been devoted to geometrically thick accretion disks and to the nuclear reactions and instabilities that may occur in them. Important unresolved issues include possible pulsational instabilities of the collimating tunnel, effects of magnetic fields on temperature and density structure, and the effects of transonic flows on stationarity and nucleosynthesis.
