Effects of topology change on low energy physics in the framework of Brans-Dicke theory of gravity is studied especially associated with the wormhole physics solution to the cosmological constant problem. It is shown that wormhole effects drive the value of observable effective cosmological constant to zero at later universes whereas to an enormous constant value large enough for the occurrence of inflationary epoch at the very early universe without such a problem as "sliding of Newton's constant problem" which occurs in Einstein gravity. Then it is explained that this pattern of wormhole-driven values of effective cosmological constant leads to the transition of the theory of dynamics for gravity from Brans-Dicke gravity at the very early universe essentially to Einstein's General Relativity at later universes. Besides, as a consistency check of the formulation of wormhole physics it is explicitly shown that the biggest difficulty associated with the wormhole physics in the context of Einstein gravity, namely "large wormhole catastrophe" does not exist in the context of Brans-Dicke gravity. In addition, it is found that the generic wormhole configuration exists even in the pure Brans-Dicke gravity for very small values of the Brans -Dicke omega parameter supporting our belief that very small values of the omega parameter represent the very early universe at quasi-quantum gravitational era.
