It is over a decade since the RXTE discovery of millisecond spins among the oldest accreting neutron stars. In that time, nearly a dozen millisecond pulsars have been discovered through persistent pulsations during X-ray outburst, with over a dozen more discovered as transient millisecond oscillations during thermonuclear X-ray bursts. Despite all we have learned from these remarkable objects, some longstanding questions remain and some unexpected new surprises have emerged. In this invited review, I will discuss both our current understanding and some open questions regarding milisecond pulsars in LMXBs, with a emphasis on the most recent work.
