Radio observations of the z = 2.29 QSO PKS 0458-020 are presented which probe the spatial extent of the gas that gives rise to damped Ly-alpha and 21 cm absorption lines at z = 2.04. Previous observations suggest an extended H I disk that would cover the large-scale structure of the background radio source, and are also consistent with smaller configurations of gas that only cover the compact core of the source. VLBI data of the line and continuum are presented which rule out small configurations such as dwarf galaxies or isolated, gravitationally bound clouds. Rather, the evidence points to an extended disklike structure with a radius of curvature larger than (8/h)kpc, where h = H(0)/100 km/s/Mpc.
