Results are presented for an identification program carried out for the 238 radio sources in the third 1.4-GHz Westerbork survey. The identifications were attempted on deep 127-04 and IIIa-J plates taken especially for this purpose with the 1.2-m Schmidt telescope at Palomar. For 85 radio sources at least one optical candidate was found closer to the radio position than three times the combined position errors. This criterion for positional coincidence allows one to estimate the number of spurious identifications. The net identification rate is 31 percent. The sample of proposed identifications has a completeness of 99 percent and a reliability of 85 percent. Of the proposed identifications 47 are with a galaxy, 16 with a stellar object, and 22 with an object of uncertain type.
