In the past few years, there has been an important shift in the theoretical understanding of low radiative efficiency accretion flows onto black holes: strong convection and strong outflows can significantly modify the structure of the accretion flow. I review these results and discuss their observational implications for low-luminosity X-ray binaries and active galactic nuclei.
