Kuijpers and Melrose make an error in imposing the symmetry
-k⇔k2. The "response" and "Lagrangian" formalisms with
the correct second symmetry -k⇔k2 agree. The most
dominant plasma maser effect comes from the polarization term, except
for the electrostatic waves not having an applied static magnetic field.
