Two principal reasons stand in the way of the space biomedical research: (1) astronauts were and most of them remain reluctant to be involved in medical investigations, claiming that no operational objectives were ever missed because of the discomforts suffered and (2) many medical researchers concluded too soon that the space related problems were largely related to terrestrial motion sickness. The paper argues for the necessity of the development of a systematic clinical characterization protocol, the use of that protocol on an adequately sized cadre of dedicated physician observer-test subjects from private research sector, and the repeated flights of that cadre to investigate the effects of countermeasures. The application of this protocol should begin by flying pairs from the physician cadre on each of the short Space Shuttle flights taking place and then reflying the cadre for longer duration, and, eventually, on the Space Station Freedom.
