High spatial resolution NTT observations of the extremely metal-poor H II galaxy SBS0335 - 052 show that the galaxy presents disturbed morphology with two nuclei and H-alpha emission extending over more than 3 kpc. The two nuclear components are separated by 1.0 arcsec and show different physical conditions. The metallicity of the most luminous component compares well with that of IZw 18, the most metal deficient galaxy known. The observations of SBS0335 - 052 combined with data for a sample of metal poor H II galaxies for which He I 6678 is detected are used to derive a value for the primordial He abundance of Y(p) = 0.216 +/- 0.006, which is marginally consistent with the value predicted by standard big bang nucleosynthesis models.
