The positions of 5C12 sources are identified and the number of sources in the survey is extended in the 4-degree diameter area near the north galactic pole. The data reduction and analysis parameters are given for both the 0.4-GHz and the 1.4-GHz survey maps, and the full source catalogue is presented including data from the corresponding frequencies. Because the number of detections with a high signal-to-noise ratio is not significantly higher in this count than in the original catalogue, a source count is not calculated. The median spectral index and low-frequency variability are calculated for the survey, and revised values for the flux density of 5C12.122 and 5C12.214 are given. The total number of new detections is 376 at 0.4 GHz and 57 at 1.4 GHz, meaning that there are now 691 sources in the area. The accurately identified sources make up a catalogue that can permit studies of radio sources and related clustering as well as the position-angle and flux-density correlations of neighboring sources.
