A census of astronomical plates in North America was conducted in the first half of 2008 under the auspices of the Working Group on the Preservation of Astronomical Heritage of the American Astronomical Society. Astronomical institutions as well as a few individual astronomers were contacted about plates they might hold using a short questionnaire distributed by e-mail. The questionnaire and the cover letter that accompanied it are presented in the following pages.
