Candidate star 12 of Cowley et al. (1977) is confirmed as the optical counterpart of 4U 1358-52. The star is a heavily reddened (B-V color excess about 2.2 mag) B0 supergiant, with absorption velocities antiphased with the X-ray source and He II 4686-A emission cophased with the X-ray source. Masses of 20 + or - 4 and 2.0 + or - 0.4 solar masses are derived for the components, with an inclination of approximately 70 deg. The He II emission originates near the X-ray source. The system is very similar to SMC X-1. It has a very high systemic velocity which may result from the presumed supernova event that formed the neutron star.
