The size distributions of the H II regions in 21 nearby spiral galaxies are determined from Hα image-tube plates. Almost all integral diameter distributions fit an exponential law, as found for irregular galaxies in Paper I. There is a strong correlation between the luminosity of a galaxy and the slope of the diameter distribution. The slope is shallower in the spiral arms and there is a weak tendency for it to be shallower also in the outer parts of a galaxy.
